INSTRUCTIONS
- From the scroll down menu below, select the Spring, Fall, Winter or Summer Sessions you want to attend.
- To expedite registration of a sibling for the same session, just add their name into the comment box with date of birth.
- If you have never submitted a Medical/Liability waiver form, please complete and mail or bring to first day of clinic.
- Press the submit button once. You will receive an automatic registration confirmation. We will also get back to you shortly with further details.
